Accident reports can be requested by mailing a self addressed stamped envelope, along with your request, to the Monroe County Sheriff’s Office, 112 S. Court Street, Room 500, Sparta, Wisconsin 54656 and including the required fee. Typical reports are charged $1.00, however, additional reports or statements are charged $.25 per page. You may contact the Sheriff’s Office by telephone to confirm the cost. The number is 608-269-2117.  Accident Reports can also be forwarded via email free of charge and Incident Reports can be forwarded via email for $.25 per page.  Please email your request to sheriff.reports@co.monroe.wi.us, or fill out the below information.  Lastly, you may request accident reports directly from the WI Department of Transportation online at The DOT website.
