Children’s Long-Term Support (CLTS)

The Children’s Long-Term Support (CLTS) Program provides services and supports to children and young adults under the age of 22 who have a disability and need support to remain in their home or community.

A functional screen is completed to determine eligibility.  Youth may qualify under three different target groups: physical disability, developmental disability, and mental health. Youth must also have a source of Medicaid to qualify but this can be provided through CLTS if needed. Parents may have a cost share based on their income, family size, and cost of services.

Referrals can be made to the Family Navigation Specialist at the Parenting Place.  Click on the link for Eligibility Based Programs - The Parenting Place, scroll down to “Family Navigation Specialist” and click on the “referral form” button.
