The Social Workers at the ADRC of Monroe County provide Information and Assistance regarding local services, resources and supports as well as Long Term Care Options Counseling
Options Counseling Referral Form
The Social Workers at the ADRC of Monroe County can provide information about the choices you have regarding decisions about where to live, what kind of help you need, where to receive the care and assistance you need, and how to pay for it.
One-on-one consultation is available to help you think through the pros and cons of the various options in light of your situation, values, resources and preferences. We also provide information regarding Wisconsin's Long Term Care programs and enrollment into those programs.
